We are partnering with a well-regarded aged care provider to recruit a Support at Home Care Partner to join their Home Care team in East Perth. This is a community-based role offering autonomy, strong support, and the opportunity to influence care outcomes for older Australians living at home. The Opportunity This role is responsible for managing a caseload of Home Care Package clients across varying levels of complexity. You will deliver end-to-end case management, ensuring services are tailored, compliant, and aligned to individual client goals and funding requirements. Key Responsibilities Manage a portfolio of clients with moderate to complex care needs Conduct initial and ongoing assessments and develop individualised care plans Support clients with budget management, service navigation, and resource allocation Deliver or oversee clinical care where applicable, including wound care and medication management Collaborate with families, allied health professionals, and external providers Provide guidance and support to care staff Ensure compliance with Aged Care Quality Standards and relevant legislation About You Demonstrated experience in Home Care Package case management Strong understanding of aged care frameworks and Support at Home reforms Ability to work independently and manage a community-based caseload Excellent communication and stakeholder engagement skills Commitment to person-centred, values-driven care Qualifications Certificate III or IV in Aged Care with strong home care experience, or Enrolled Nurse or Registered Nurse (higher rate applicable) What's on Offer Salary packaging Autonomous role with strong organisational support Opportunity to contribute to a growing service during a period of sector change If you are looking for a role where you can take ownership of your portfolio and play a key role in delivering high-quality community care, we would love to hear from you!
